📋 Description
• Conduct housing assessments and determine client eligibility for housing supports • Assist clients with housing search, applications, and move-in coordination • Build relationships with landlords and local housing programs • Support clients in maintaining lease compliance and resolving tenancy issues • Provide ongoing tenancy-sustaining services (budgeting, life skills, advocacy) • Collaborate with MCO housing teams and Grace Community Care’s intake and billing departments • Maintain accurate documentation and case notes in Therap/HMIS • Participate in regular team meetings and training
🎯 Requirements
• Experience in housing navigation, case management, or social services required • Knowledge of NJ FamilyCare/Medicaid, DDD, or Section 8 preferred • Strong communication and organizational skills • Valid NJ driver’s license and reliable transportation • Ability to travel locally for home visits or landlord meetings
