📋 Description

• Conduct outbound calls, texts, and other virtual outreach to schedule Annual Visits and follow-up appointments with existing patients • Maintain a daily outbound call target of 135+ calls, working through structured call queues in our internal CRM tools throughout the day • Send SMS and message-based outreach to patients to confirm appointments and follow up on scheduling • Navigate and maintain records across internal CRM systems and spreadsheets simultaneously during and after calls • Educate patients on the importance of ongoing care and the benefits of completing their annual visits and any necessary follow-up appointments with clinicians • Handle calls from patients with warmth and professionalism, answering questions and directing them appropriately • Collaborate closely with clinical care teams to ensure alignment on visit readiness and follow-ups • Provide an empathetic, respectful, and encouraging patient experience on every call • Monitor scheduling metrics and contribute to achieving engagement goals, maintaining both call volume targets and service quality • Act as a liaison between patients and the care team to address basic concerns and questions about services

🎯 Requirements

• High School Diploma/GED or higher • 2+ years of related experience in a high-volume outbound call environment, preferably in healthcare customer service or healthcare sales/direct marketing • Excellent patient communication skills, with the ability to quickly build trust and rapport with seniors and other patients • Strong computer skills and basic knowledge of G-Suite (e.g., Google Docs/Sheets) • Can handle multiple tasks at once, such as typing notes while talking and working in 2 systems at the same time • Experience working in an Electronic Medical Record (EMR) system (Preferred) • Knowledge of Medicaid and/or Medicare programs and senior services (Preferred)
