📋 Description

• Accountable for driving market-wide quality and savings targets by supporting the development of strategies and overseeing the execution of core ACO transformation strategies. • Analyze market-level performance gaps and variances to understand root causes and implement corrective actions. • Align market activities with internal teams (Performance, Product, etc.) to ensure practice transformation goals are met. • Lead a team of Practice Transformation Specialists (PTS) to ensure high-quality service delivery within the Explorer Market. • Provide guidance on structured intervention attempts for practices while managing day-to-day field requests. • Support Aledade’s People Management strategy for the Explorer team. • Provide frontline feedback to the Program Director to help build and refine the service delivery model while standing up the program. • Mitigate risks and prioritize initiatives that support the evolution of the virtual service model. • Cultivate internal partnerships to align strategic priorities and identify opportunities for program growth and savings.

🎯 Requirements

• 6-8 years of experience in Value-Based Care (VBC), with a proven track record of driving operational performance and delivering successful quality/financial outcomes. • 6-8 years of experience utilizing healthcare data analytics to uncover strategic insights, solve complex problems, and improve performance outcomes. • 3–5 years of experience managing a structured team (minimum of 3+ direct reports) • Results-oriented leader with a demonstrated ability to prioritize and manage multiple complex projects simultaneously.
