📋 Description

• Provide compassionate and skilled nursing care to adult patients in the comfort of their homes. • Assess patients' needs, develop care plans, and coordinate with other healthcare professionals. • Conduct comprehensive assessments of patients' physical, emotional, and social needs. • Educate patients and their families on disease management, medication administration, and self-care techniques. • Provide skilled nursing care including wound care, medication management, IV therapy, catheter care, and other treatments as prescribed. • Collaborate with physicians, therapists, social workers, and other healthcare professionals. • Maintain timely, accurate documentation of assessments, care provided, and patient outcomes in Kinnser/Wellsky (EHR).

🎯 Requirements

• Current and unrestricted Registered Nurse (RN) license valid in the State of Texas. • Minimum of one year of nursing experience in adult home health. • Experienced in all OASIS tasks: SOC, ROC, Recertification, Discharge and regular visits. • Experience with Kinnser/Wellsky a plus, additional training on Kinnser can be provided by our team. • Strong clinical assessment and critical thinking skills to effectively address patients' complex needs. • Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals. • Ability to work autonomously, prioritize tasks, and manage time efficiently in a dynamic home health environment. • Willingness to travel to patients' homes within the designated service area and have reliable transportation. • BLS (Basic Life Support) certification.
