• Conducting advisory consultations (according to §37 para. 3 SGB XI) with insured persons and their relatives in the home environment • Documenting consultations, identifying individual needs, and initiating appropriate follow-up measures • Training caregiving relatives according to §45 SGB XI • Advising care-dependent persons and relatives seeking guidance as part of care counseling according to §7a SGB XI • Conducting product trainings and providing professional advice for employees of various companies to sustainably improve the compatibility of caregiving and work • Supporting the design and creation of informational materials • Establishing and expanding regional networks and partnerships
• Completed training as a Registered Nurse, Geriatric Nurse, Nursing Specialist, or equivalent qualification • At least two years of professional experience • At least six months of experience in outpatient/home care • Qualifications in accordance with §45 and/or §7a SGB XI or willingness to obtain them • Proficient use of MS Office and modern digital work tools • Strong communication skills to support better reconciliation of work and caregiving responsibilities • Driver's license class B
