• Diagnose and treat patients who have health conditions that limit their ability to move and perform everyday activities • Help restore their physical function and mobility, promote overall wellness and boost a patient’s quality of life • Teach different functional tasks and exercises intended to strengthen or stretch muscles as well as alleviate pain • Reviewing patients’ medical histories • Developing individualized care plans for patients • Outlining clear goals for patients and the expected outcomes of the plan • Using exercises, stretching, equipment, and hands-on therapy to manage patients’ pain, increase mobility, and prevent further pain and injury • Recording patients’ progress and modifying the plan of care if needed • Educating patients and family members about the recovery process • Administering medically prescribed physical therapy treatments to relieve pain and improve mobility • Advising patients on exercise techniques • Providing educational information about injury prevention, ergonomics, and ways to promote physical health • Consulting and collaborating with other healthcare professionals within the agency • Documenting patient care history • Complying with rules, regulations, and procedures of the agency
• Bachelor’s degree in Physical Therapy • Philadelphia Physical Therapy (PT) license and CPR Certification • Strong Clinical & Interpersonal Skills • Excellent Written & Oral Communication Skills • Strong ability to work independently • Flexibility to change care plans if the patient is not getting the best results • Superior organizational skills to manage several different cases at once • Computer literacy to maintain and manage case records • Compassionate and attentive to detail • Possess up-to-date knowledge of treatment practices
