📋 Description

• Provide clinical expertise to support patients navigating chronic illness. • Partner with healthcare navigators to build and oversee holistic care plans. • Identify and prioritize patient needs, ensuring continuity of care. • Integrate social determinants of health (e.g., food, transportation, housing) into patient plans. • Contribute to building Aviator’s new patient care program, helping us refine and expand our model. • Use technology and AI-enabled systems to deliver care efficiently and effectively.

🎯 Requirements

• Active member of an IMLC state and open to acquiring additional licensure. IMLC member states are AL, AZ, CO, CT, DE, DC, FL, GA, HI, ID, IL, IN, IA, KS, KY, LA, ME, MD, MI, MN, MS, MO, MT, NE, NV, NH, NJ, ND, OH, OK, PA, SD, TN, TX, UT, VT, WA, WV, WI, WY, GU • Currently licensed in Texas (TX), Florida (FL), and Georgia (GA) - Ideally licensed in all 50 states. • Board certification in Internal Medicine, Family Medicine, or related specialty. • Experience caring for patients with chronic conditions. • Strong clinical judgment and ability to coordinate across multiple providers. • Excellent communication and empathy skills; you listen first and guide second. • Comfortable with technology and able to quickly learn new systems. • A proactive, solutions-oriented mindset; you thrive in fast-paced, evolving environments.
