📋 Description

• Engage with patients through in-app messaging, phone calls, and telehealth check-ins to support adherence to their care plans. • Monitor patient data (such as blood pressure readings) and conduct follow-ups with patients who have not submitted readings in several days. • Educate patients on how to use digital health tools and properly measure vital signs. • Escalate concerns related to abnormal patient readings or reported symptoms to RN Care Managers or clinical teams based on established protocols. • Provide care coordination support, including appointment scheduling and answering routine patient questions. • Assist in patient onboarding for Babyscripts' RPM program, ensuring patients understand expectations and have access to necessary resources. • Document all interactions accurately in Babyscripts' care management platform.

🎯 Requirements

• Experience in care coordination, patient engagement, or remote patient monitoring (RPM) is highly preferred. • Background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role. • Strong communication and interpersonal skills, with a patient-first mindset. • Proficient with digital health tools, EHR systems, and telehealth platforms. • Ability to work independently in a remote environment while maintaining productivity. • Comfortable with structured but flexible scheduling, ensuring full-time work commitment. • Bilingual (Spanish preferred) is a plus.
