📋 Description
• Provide telephonic complex case management • Partner with members, caregivers, providers, and community resources • Conduct comprehensive assessment and individualized care planning • Collaborate with interdisciplinary teams to promote better health outcomes • Facilitate seamless transitions across the continuum of care
🎯 Requirements
• Current Registered Nurse (R.N.) license in state worked • Bachelor's degree in case management or health care • 3-5 years clinical experience • Basic Life Support (BLS) certification for acute care settings • Knowledge of care management and community resources • Critical thinking and problem-solving skills • Ability to work effectively in an interdisciplinary team format
