📋 Description
• Begeleiden en ondersteunen van cliënten binnen de specialistische GGZ • Voeren van online gesprekken en in kaart brengen van de psychosociale situatie van cliënten • Signaleren van risico's en bewaken van de voortgang van behandeltrajecten • Nauw samenwerken met regiebehandelaren, psychiaters, psychologen en andere collega's binnen het multidisciplinaire behandelteam • Bijdragen aan de kwaliteit en continuïteit van de zorg door cliënten te begeleiden, risico's te signaleren en behandelaren te ondersteunen bij complexe casuïstiek
🎯 Requirements
• Een afgeronde opleiding HBO-Verpleegkunde • Een geldige BIG-registratie als verpleegkundige • Een afgeronde post-hbo opleiding tot Sociaal Psychiatrisch Verpleegkundige (SPV) • Minimaal twee jaar relevante werkervaring binnen de specialistische GGZ • Kennis van psychiatrische ziektebeelden, risicotaxatie, crisisinterventie en herstelgerichte zorg • Beheersing van de Nederlandse taal uitstekend, zowel mondeling als schriftelijk • Beheersing van de Engelse taal een pre • Ingeschreven als ZZP'er bij de Kamer van Koophandel • Geldige beroeps- en bedrijfsaansprakelijkheidsverzekering • Bereid om een geldige Verklaring Omtrent het Gedrag (VOG) aan te leveren
