• Collaborates with multidisciplinary care providers for remote services • Reviews and interprets patient medication orders remotely • Applies pharmacotherapeutic and pharmacokinetic principles for medication use • Provides drug information and consultation to healthcare professionals • Engages with other clinical pharmacists for complex clinical situations • Participates in quality improvement activities and medication management initiatives
• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy • Current unrestricted licensure as a licensed pharmacist in the State of Rhode Island • Current licensure as a registered pharmacist in the Commonwealth of Massachusetts • Within 18 months of hire, Board of Pharmacy Specialties certification in clinical pharmacy expertise • Successful completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or a minimum of 3 years clinical experience • Effective interpersonal and communication skills, both written and verbal • Knowledge of pharmacy information systems and automated dispensing technology. • Experience with MS Word, Excel, Outlook, and web browser.
