📋 Description
• The Consultative Pharmacist 2 consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy • Advises safe, accurate and timely usage of medications • Designs, implements, evaluates and makes adjustments to medication care plans • Monitors medication therapy and makes adjustments as needed • Works as the medication expert in focused proactive heath campaigns that target improving health outcomes for patients with specific conditions
🎯 Requirements
• Bachelors Degree • Pharmacist license in good standing in OH, AZ, Fl or TX • Interact with CenterWell Pharmacy patients via the telephone • Provide counseling and advice • Research patient concerns • Facilitate positive service and pharmaceutical outcomes
