📋 Description
• consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy • advises safe, accurate and timely usage of medications • designs, implements, evaluates and makes adjustments to medication care plans • monitors medication therapy and makes adjustments as needed • works as the medication expert in focused proactive health campaigns that target improving health outcomes for patients with specific conditions • begins to influence department’s strategy • makes decisions on moderately complex to complex issues regarding technical approach for project components
🎯 Requirements
• Bachelor's degree • Must have an active license with the Board of Pharmacy in the appropriate state • Ability to participate in Federal prescription programs • Ability to solve problems and encourage others in collaborative problem solving • Work ethic that is focused, accurate, and highly productive • Proficient with computers • PharmD preferred • 3 plus years' experience in a high volume community or mail order pharmacy practice environment
