âą Use evidence-based medicine and clinical judgment to evaluate pharmacotherapy suitability for high-risk patients âą Execute clinical protocols as allowed per organization and state regulatory agencies âą Optimize drug coverage benefits for appropriate care and reduced costs âą Collaborate with healthcare professionals to optimize patient care âą Participate in high-risk rounds to discuss patient progress âą Document all activity within clinic EMR and other required systems âą Conduct in-service education sessions for healthcare staff
đŻ Requirements
âą Active and unrestricted Florida pharmacist license âą Willingness to become licensed in additional states for market support âą Pharm.D. or other advanced pharmacy degree âą Bilingual in English and Spanish with the ability to read/write/speak fluently in both languages âą 5+ years' experience as a practicing pharmacist âą Experience working in a team environment and supporting common goals
