📋 Description

• Provides fully remote virtual serious illness care management to patients and families • Performs virtual assessment of patient/family/caregiver needs • Identifies, manages, orders, and prescribes medication and equipment • Adjusts and supports the plan of care in accordance with policies and procedures • Supports staff by answering clinical questions and supporting the clinical needs of patients

🎯 Requirements

• 3 years of recent clinical experience • Experience in hospice, palliative care, and/or acute inpatient care • Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program • National certification, licensure, and prescriptive authority from the governing body for advanced nursing practice in their home state • Basic Life Support certificate from an approved organization
