📋 Description
• Provides non-face-to-face telephone triage, care, and planning to Medicare patients • Collaborate with healthcare team to optimize care • Educate and coach patients in reaching health goals • Communicate with patients and providers to coordinate care services
🎯 Requirements
• Fluent/Bilingual in Spanish • Current COMPACT license as an RN • Willing to obtain multi-state licenses of ChartSpan footprint • Familiar with chronic care conditions and medications such as HTN, CHF, COPD, DM • Excellent computer skills and ability to multitask
