📋 Description
• Provide comprehensive psychiatric evaluations and ongoing medication management via telehealth • Treat a diverse adult and geriatric patient population with varying psychiatric conditions • Develop individualized, evidence-based care plans tailored to patient needs • Prescribe and adjust medications with careful monitoring and follow-up • Maintain continuity of care through consistent patient engagement • Collaborate with clinical and support teams to ensure coordinated outpatient care • Document encounters accurately and in compliance with clinical standards • Educate patients and caregivers on diagnoses, treatment options, and medication plans
🎯 Requirements
• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP) • Active and unrestricted New York State Nurse Practitioner license • Ability to work full-time aligned with Eastern Time clinic hours • Experience working with adult and/or geriatric populations • Prior outpatient or telehealth experience is a plus • Comfort managing a steady outpatient caseload independently • Strong communication skills and ability to build rapport in a virtual setting • Proficiency with telehealth technology and EMR systems • Bilingual English/Spanish strongly preferred.
