📋 Description

• Coordinate care for members with chronic medical and behavioral health conditions • Conduct comprehensive assessments to evaluate members’ needs • Develop and implement individualized care plans • Provide disease management education and support • Ensure interdisciplinary team collaboration for optimal health outcomes • Maintain meticulous documentation of care management activities

🎯 Requirements

• Must have active and unrestricted Registered Nurse (RN) licensure in the state of MO OR compact licensure in state of residence • Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • Access to a private, dedicated space to conduct work effectively • Minimum 3+ years of nursing experience • Minimum 2+ years of case management, discharge planning and/or home healthcare coordination experience • Experience providing care management for Medicare and/or Medicaid members (preferred) • Bilingual skills, especially English-Spanish (preferred)
