📋 Description
• Provide In-Home Health Evaluations, engaging with people in their homes • Building trusted relationships to make people healthier • Conduct virtual visits as needed • Provide all Signify Health services as indicated including Diagnostic & Preventive Services products
🎯 Requirements
• Active, unrestricted license(s) in coverage area(s) • Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting (required, as is the filing of the appropriate attestation with the state of Minnesota) • Board certification (required) • Multi-state licenses (preferred)
