• Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes • Travel locally and within licensed states • Conduct virtual visits as needed • Provide all Signify Health services as indicated including Diagnostic & Preventive Services products

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required
