• Assess clients’ needs, strengths, and goals to determine eligibility • Develop and regularly review Individualized Service Plans (ISPs) • Coordinate multidisciplinary care and maintain communication with stakeholders • Ensure all documentation is accurate and compliant • Monitor progress and adapt plans as needed • Uphold Medicaid waiver program requirements • Advocate for clients within healthcare and community systems • Participate in training and quality improvement initiatives • Collaborate with providers to deliver holistic support
• Bachelor’s degree in Social Work, Psychology, Human Services, or related field • 3+ years of case management experience (preferably with developmental disabilities or Medicaid waivers) • Knowledge of Indiana waiver programs (HCBS), Medicaid regulations, and compliance standards • Proficiency with EHR/case management software • Excellent communication and organizational skills • Valid driver’s license, reliable transportation • Experience with person-centered and trauma-informed care • Ability to manage multiple caseloads, prioritize, and meet deadlines • Commitment to ethical practice and confidentiality
