📋 Description

• Provide virtual care primarily via CBT to patients diagnosed with cancer • Conduct initial needs assessments for patients • Collaborate with Health Partners, Nurses, and Nurse Practitioners • Monitor side effects and changes in patient’s function or behavior • Consult with licensed clinical team members for complex issues • Document patient interactions in EHR system • Integrate collaborative care programming into Daymark care model

🎯 Requirements

• Bachelor's degree and Master of Social Work (MSW) or MSW-equivalent program • Active Licensed Clinical Social Work (LCSW) license • At least two years of clinical work experience • Adept at using CBT or similar techniques • Working knowledge of medical terminology • Licensed (or willing to be licensed) in Rhode Island, Massachusetts, and Connecticut • Comfortable using Slack, Zoom, Google Suite, and EHR systems • Ability to establish rapport with diverse populations • Organized, communicative, and professional in writing
