📋 Description
• Provide expert clinical care for the Palliative Care Program • Direct supervision of palliative care clinical staff • Participate in referral management, process improvement, and change management • Provide direct patient care to patients enrolled in the palliative care program by phone or video visit during scheduled and on-demand patient visits • Participate in and/or lead patient panel reviews • Model and support development and implementation of new workflows during rapid cycles of iteration • Create performance improvement plans, when needed • Perform regular clinical audits (QA) that include a mix of note review and listening tours
🎯 Requirements
• 5+ years of experience as a Palliative Care Clinician • 2+ years of experience leading front line clinical teams with accountability for team level outcomes • Demonstrated ability to champion new initiatives and achieve direct business outcomes • Great clinical and non-clinical judgment and ability to coach around performance to drive clinical effectiveness • Deeply empathetic and humanistic, committed to the mission of providing quality care.
