• In close and trusting cooperation with the facility management, you will organize and conduct consultation appointments on-site. • Your advisory approach is self-reflective, responsible, and respectful. • You conscientiously and reliably document consultation processes and expressions of will (advance directives) and communicate this information to the nursing and care teams. • Thanks to your cooperative and empathetic manner, you find it easy to raise staff awareness about responsibly ensuring the implementation of expressed wishes. • You are communicative and enjoy structured networking and collaboration with regional providers involved in health care planning, such as physicians, emergency and hospice services, and other institutions.
• Completed vocational training as a nursing professional or a relevant degree in the health sector. • Three years of professional experience within the last eight years in inpatient or outpatient care facilities or in a hospice. • Further training/certification as an advisor for health care planning for the last phase of life in accordance with § 132g SGB V (German Social Code Book V). • Strong social skills and empathy.
