📋 Description

• Support multiple hospitals each with a daily census of 30-60 patients. • Use ACEIT and various medication processes to assess, plan, intervene when necessary, implement and evaluate patient care in compliance with hospitals' policies, protocols, and procedures. • Manage generated protected health information in a manner consistent with the privacy rule as outlined in HIPAA. • Document clinical intervention and comply with individual hospital reporting requirements. • Review drug orders, formulary compliance, and evaluate reported lab values to determine appropriateness of medication therapy.

🎯 Requirements

• Licensed in good standing by the State Pharmacy Board. • Successful completion of a degree from an accredited pharmacy program or one that is determined to be substantially equivalent to a U.S. accredited program by an appropriate certifying agency. • Minimum 1 year of supervisory experience.
