📋 Description

• Review of the admission status and documentation of admission diagnoses • Conducting assessments to determine whether a case qualifies for case management • Initiation and monitoring of discharge management (interface coordination) • Responsibility for length-of-stay management • Case-related primary coding of diagnoses as well as monitoring and completion of OPS (procedure coding)

🎯 Requirements

• Completed training as a registered nurse (m/f/d) • Completed further training as a Case Manager • Professional experience, particularly in Internal Medicine • Advisory and organizational skills • Good knowledge of coding nursing-relevant secondary diagnoses and procedures • Enjoyment of working in an interdisciplinary team • High level of commitment and assertiveness • Patient-oriented and independent working style
