• Contact patients with abnormal screening results to discuss findings and provide guidance on next steps. • Deliver clear and concise information about the nature of the abnormalities and potential health implications. • Develop and communicate personalized care plans, including necessary follow-up actions and preventive measures. • Collaborate with a multidisciplinary team to ensure comprehensive patient care. • Maintain accurate and timely documentation of patient interactions and care plans.
🎯 Requirements
• Active Nurse Practitioner license in the state of Michigan. • Minimum of 2 years of clinical experience, preferably in a preventative care setting. • Strong communication and interpersonal skills. • Ability to deliver sensitive information with empathy and professionalism. • Proficient in electronic health records (EHR) systems.
