📋 Description

• Oversee RTM care navigation operations, team performance, workflow execution, and continuous process improvement. • Support operational consistency, documentation quality, and billing readiness. • Collaborate with Clinical, Concierge, Billing, Product, and Data teams to support scalable, high-quality RTM operations. • Manage, coach, and develop RTM Care Navigation employees and contractors. • Monitor workflow adherence and identify operational risks affecting patient experience. • Track, analyze, and report on RTM operational KPIs.

🎯 Requirements

• Must be a Licensed Physical Therapist with experience in outpatient, home-based, orthopedic, post-acute, or rehabilitation care settings. • Must have a minimum of 3 years of management experience in clinical operations, rehabilitation operations, care coordination, or a healthcare environment. • Must have experience managing teams, workflows, and operational processes within a healthcare or clinical operations setting. • Strong understanding of healthcare workflows, documentation requirements, patient follow-up processes, and compliance standards. • Data-driven mindset with experience using dashboards, KPIs, and operational performance metrics. • Excellent organizational, communication, follow-up, and problem-solving skills. • Ability to thrive in a fast-paced environment while promoting structure, consistency, accountability, and operational excellence. • **Additional Preferred Qualifications** • Experience with Remote Therapeutic Monitoring (RTM), Remote Patient Monitoring (RPM), care navigation, or app-based patient engagement programs. • Familiarity with CPT-based workflows, healthcare billing concepts, or documentation requirements related to reimbursable clinical services. • Experience working with digital health platforms, app-based care delivery models, or remote patient support programs.
