📋 Description
• Plans and manages pediatric medical and/or behavioral care • Develops a member centric plan of care • Documents all encounters with pediatric members • Coordinates referrals to assist in goal achievement
🎯 Requirements
• NYS RN or LCSW, LMSW • 1 year of pediatric clinical field experience • Ability to collaborate with multiple stakeholders • Proficient with navigating multiple electronic documentation systems
