📋 Description

• Conduct comprehensive telehealth assessments for newly enrolled Medicare clients • Synthesize complex medical histories and identify clinical priorities for ongoing care coordination • Document clinical findings that inform care manager (Hero) intervention strategies • Provide clinical oversight and collaboration as needed with care management team • Establish clinical credibility and trust during initial patient contact

🎯 Requirements

• MD or DO (must be licensed to practice in: MD, PA, MA, CA, FL, OH, WA, CO, MI, NC, GA, AZ, NJ) • Substantial experience in geriatric medicine, primary care, or chronic disease management • Confident in telehealth assessment and skilled at building clinical rapport across a screen • Strong clinical judgment and comfort with complexity and ambiguity • Aligned with our mission: you see older adults as deserving of excellent, attentive care
