📋 Description
• Manage patients’ specialized needs in collaboration with healthcare professionals • Take full ownership of assigned patients with complex chronic conditions • Develop comprehensive plans of care for each member • Coordinate patient care delivery with clinicians and various services • Build relationships with patients and families • Identify patients’ needs based on values and goals • Implement care/treatment plans by coordinating access to services • Monitor care and identify cost-effective measures • Facilitate patient admissions and discharges from facilities • Act as patient advocate and educator • Develop individualized education plans for patients/families • Participate in data collection and analysis of clinical outcomes • Represent Honest Health positively to patients and families
🎯 Requirements
• Must have reliable access to high-speed internet • Active Registered Nurse license in New York • Willing to obtain additional state nursing licenses • Experience engaging patients virtually using different technologies • Bachelor’s of Science in Nursing preferred • Certified Case Manager (CCM) credential preferred • 3+ years of clinical practice in a healthcare setting preferred • Case management experience with a senior population preferred • Disease management and/or physician office experience preferred • Prior experience with electronic health records required • Microsoft Office skills including Excel, Word, PowerPoint, Outlook required
