📋 Description
• Delivering virtual cardiac rehabilitation coaching and education. • Conducting assessments and developing personalised care plans. • Monitoring participant progress and managing clinical risks. • Supporting participants to engage with the Cardihab app to support their rehab. • Communicating with GPs, specialists and other healthcare providers to coordinate care.
🎯 Requirements
• At least 5 years professional clinical experience as either a Registered Nurse, Exercise Physiologist or Physiotherapist with current registration with AHPRA or ESSA • 2+ years’ experience delivering cardiac rehabilitation (phase 2) within the past 5 years. • Previous health coaching experience and understanding of health behaviour change methodology. • Experience in acute and primary health care and care planning. • Solid understanding of cardiac rehab programs, cardiac disease, and evidence-based practice.
