📋 Description
• Provide 1:1 virtual counselling to patients enrolled in our 12-week Lifestyle Medicine program • Support patients in navigating behaviour change, emotional eating, stress, motivation, and other Psychological aspects of mental health and health improvement. • Develop and deliver care plans to help patients improve sleep and stress resilience • Collaborate with a multidisciplinary team, including physicians, dietitians, and physiotherapists • Participate in training to become confident in delivering care using our lifestyle medicine framework
🎯 Requirements
• Registered to practice as an RCC or RP in Ontario • Training and/or experience in behavioural change, CBT, ACT, motivational interviewing, and lifestyle medicine • Strong communication and interpersonal skills • Comfortable with technology and delivering care in a virtual environment • Able to work independently and within a collaborative team setting
