📋 Description

• Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s home • Ensure caregivers have the current and appropriate patient Plan of Care • Email or securely transmit care plans and related documentation to caregivers when necessary • Document all communication, follow-ups, and care coordination activities accurately in the EMR system • Escalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisor • Maintain HIPAA compliance and patient confidentiality at all times • Support continuity of care and timely coordination between field staff and office personnel • Assist with basic care coordination and administrative nursing tasks as assigned

🎯 Requirements

• Registered Nurse with New York License - USRN - NY • Fluency in Cantonese and/or Mandarin and English required • Previous experience in Home Health, Care Coordination, Case Management, or Remote Nursing preferred • Strong verbal and written communication skills • Experience using EMR/EHR systems • Excellent organizational and documentation skills • Ability to work independently in a remote environment • Proficient with email, phone communication, and basic computer systems
