📋 Description

• Conduct virtual patient assessments and medical histories • Diagnose and manage obesity and related chronic conditions • Work closely with patients’ families in a family-based therapy model • Prescribe and manage medications in accordance with clinical protocols and supervision requirements • Develop, implement, and adjust individualized treatment plans • Monitor patient progress, safety, and treatment response • Educate patients on medical and behavioral aspects of obesity care • Escalate clinical concerns and adjust care as appropriate • Collaborate with interdisciplinary care teams and participate in case reviews • Maintain accurate, timely clinical documentation in the EHR • Uphold telehealth, prescribing, and compliance standards

🎯 Requirements

• Active PA or NP license in good standing (required) • Connecticut licensure & DEA (required) • NP board certified or PA program graduate (accredited) • Pediatric/adolescent primary care, obesity medicine, endocrinology, or chronic disease experience (preferred) • Telehealth experience (preferred) • Spanish-English bilingual capability is a plus • EHR and telehealth platform proficiency
