📋 Description
• Support Biologics patients through virtual assistance • Assist patients and their caregivers in a non-clinical capacity • Provide community resource support and health literacy sessions • Coordinate and lead monthly peer support groups • Ensure accurate data entry in the system of record
🎯 Requirements
• Minimum 3 years of relevant experience • High School Diploma required • CHW certification or training preferred • Proficiency in Microsoft Office Suite (Outlook, Excel, Word, Teams, PowerPoint) • Strong interpersonal and communication skills • Experience in community outreach and health advocacy with direct patient interaction • Ability to guide and support individuals in accessing health resources • Problem-solving and decision-making skills • Highly organized and proactive • Flexible availability for multiple shifts and occasional travel
