📋 Description

• Create and track patients' care plans • Accompany patients in their healthcare journey • Regularly engage with patients and track their adherence to their care plans. • Closely monitor patients' device readings and dig into any out-of-bound values. • Maintain meticulous records of patient interactions and program details. • Stay up-to-date with the latest regulations and compliance standards related to CCM/RPM services. • Ensure all patient enrollments adhere to legal and ethical standards. • Monitor and track care performance, striving to meet or exceed goals.

🎯 Requirements

• National Certification in Medical Assisting • Residence in the US • Strong communication skills, both written and verbal. • Empathy and the ability to connect with patients and their families. • Basic knowledge of chronic health conditions and their management. • Comfortable using technology for documentation and communication. • Attention to detail and strong organizational skills. • Ability to work independently and as part of a team.
