📋 Description

• Finding resources and services for clients that best fit their needs • Advocating for persons with disabilities/elderly • Carrying caseloads consisting of persons with intellectual disabilities, traumatic brain injuries, CADI diagnoses, and elderly/alternative care • Visiting persons on the caseload and attending team meetings • Completing needs assessment; individual service plan development (CSSP) • Assisting with planning for new service development • Locating residential, vocational, and other needed services • Monitoring service delivery and ensuring the health and safety needs of each person are being met • Completion of referrals and crisis intervention as needed • Developing waiver budgets • Completing case notes

🎯 Requirements

• Be a licensed Social Worker (Bachelor of Social Work required for licensure) • Or licensed Graduate Social Worker (Master’s in Social Work required for licensure) as stated by the Minnesota Board of Social Work • Or have a 4-year degree in Human Services, Psychology or Sociology, or related fields • Successful clearance of Department of Human Services background check • Successful clearance of Motor Vehicle Background Check and acceptable driving record per Company Policy
