• Lead the Population Health Management Program (SPHMP), expanding the program and delivering on all financial and clinical goals • Understand contractual obligations to the health care plan partner, in full, and ensure that all are met successfully at all stages of program implementation and delivery • Create a plan to implement new programs, in partnership with the GM of Population & Speciality Health and the implementation team • Solve problems throughout implementation to ensure goals are met and health plan partners are satisfied • Deliver against monthly and quarterly enrollment objectives through SMART metrics and KPI’s • Elevate team performance by providing comprehensive coaching and mentorship • Ensure all team members know how to perform core functions of their job • Collect and synthesize data to support team training, improve high-quality engagement, and deepen internal understanding • Analyze team activities, inputs, and outcomes to develop continuous improvement plans • Partner with People Operations to hire, coach and mentor an exceptional team • Manage and report on operational budgets for the program • Identify Member’s behavioral health diagnoses and ensure comprehensive service plans • Partner with the billing team to review and approve claims • Ensure compliance with all local, state, federal laws, and health plan rules and regulations • Lead internal chart review for partner audits • Participate in a rotating on-call schedule to support members.
• A Qualified Mental Health Professional (QMHP) holding one of the following clinical licenses is required: Licensed Clinical Social Worker (LCSW), Clinical Professional Counselor (CPC), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist (PhD or PsyD), or equivalent • 10+ years of relevant population health management experience • Experience leading, building and implementing programs nationally with strong outcomes • Master's degree in behavioral health, social work or health care management from an accredited university required • Experience in Health Coaching or Medical Social work strongly preferred.
