📋 Description

• Support chronically ill patients through advanced illness management, palliative care management, and hospice coordination. • Triage urgent palliative care needs and escalate appropriately. • Provide clinical support to regional palliative physicians, including documentation and workflow assistance. • Facilitate hospice referrals and coordinate informational visits. • Support scheduling and pre-visit education for complex patients. • Obtain ACP documentation from external offices and hospitals.

🎯 Requirements

• Graduate of an accredited School of Nursing • Currently licensed as a Registered Nurse • 2+ years of experience working in palliative-focused care, geriatric home-based care, and/or hospice care • Ability to travel to assigned market(s) up to 20% • Self-starter with the ability to work independently and collaboratively in a remote environment with minimal supervision • Familiarity with Microsoft Office and web-based applications • Excellent verbal and written communication skills • Prior experience in value-based care models preferred
