📋 Description

• Guide patients through enrollment, onboarding, and transitions into the Functional Psychiatry Membership Program. • Serve as the primary administrative point of contact, providing support and education throughout the patient journey. • Monitor patient engagement and adherence to treatment recommendations, conducting follow-ups as needed. • Coordinate with providers and cross-functional teams to ensure seamless care transitions and continuity of care. • Maintain accurate documentation and track patient progress, enrollment activities, and program participation. • Address and escalate billing, insurance, or clinical concerns to the appropriate teams. • Support program growth and quality improvement initiatives by identifying barriers and enhancing the patient experience.

🎯 Requirements

• Active RN license and experience working in a mental health or behavioral healthcare setting. • Strong patient coordination, communication, and customer service skills. • Ability to build rapport with patients and support them throughout their care journey. • Excellent organizational, time management, and multitasking abilities. • Strong critical thinking and problem-solving skills with keen attention to detail. • Ability to collaborate effectively with providers and cross-functional teams. • Proficiency with Microsoft Office, Google Workspace, and other computer-based systems. • A proactive, patient-centered approach with a commitment to delivering a positive patient experience. • Ability to work independently in a fully remote environment while maintaining productivity and accountability. • Alignment with the organization's values of excellence, adaptability, positivity, transparency, and continuous learning.
