📋 Description
• Provide comprehensive Medication Therapy Management (MTM) services • Conduct medication reconciliation and assess therapy effectiveness • Deliver patient-focused education on medications and chronic conditions • Support chronic disease management (e.g., diabetes, hypertension, COPD, CHF) • Utilize motivational interviewing techniques to drive adherence and engagement • Collaborate with providers to optimize medication regimens and care plans • Identify and resolve medication-related issues and risks • Document patient interactions accurately and in compliance with program standards • Support care transitions and continuity of care initiatives
🎯 Requirements
• Doctor of Pharmacy (PharmD) • Active pharmacist license in Hawaii (HI) • Willingness to obtain additional state licensure if needed • Experience in Medication Therapy Management (MTM) • Experience in ambulatory care or outpatient settings • Experience in chronic disease management • Experience in patient education and engagement • Familiarity with care programs such as CCM, TCM, and RPM is a plus
