📋 Description

• Conduct virtual musculoskeletal triage and follow-up visits to assess symptoms, functional limitations, and clinical risk. • Apply strong clinical reasoning to determine appropriate clinical recommendations within a virtual care model. • Deliver evidence-based virtual physical therapy interventions focused on functional improvement and symptom management. • Develop individualized care plans aligned with patient goals, functional needs, and long-term outcomes. • Identify clinical needs requiring care coordination and route tasks to Care Coordinators. • Communicate and coordinate with patients, care coordinators, APPs, and external referral sources to support continuity across the episode of care. • Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory, payer, and organizational requirements. • Adhere to established clinical protocols, escalation pathways, and quality standards within a value-based care environment.

🎯 Requirements

• Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program. • Active, unrestricted physical therapy license in the state(s) where care will be delivered. • Minimum of 3 years of clinical experience, with preference for 5 years. • Strong clinical reasoning skills and a patient-first approach to care. • Comfort practicing in a virtual environment, including remote evaluations and video-based patient education. • APTA specialist certification (e.g., OCS, SCS) preferred. • Residence in PT Compact State. • Experience delivering care in virtual or telehealth-based models. • Experience working in interdisciplinary or integrated care teams alongside medical providers. • Spanish language proficiency preferred.
