📋 Description

• Conduct comprehensive telehealth assessments for older adults with chronic and complex conditions • Identify clinical risks and gaps in care across diagnoses, medications, and recent utilization • Oversee patient care plans in partnership with healthcare advocates, addressing both medical needs and social determinants of health • Collaborate with therapists and care teams to ensure physical health is fully integrated into ongoing care

🎯 Requirements

• MD or DO, licensed in Florida, Maryland, Pennsylvania, and Texas and additional states preferred • Board certification in Internal Medicine, Family Medicine, or related specialty • At least 5 hours of availability a week • Experience caring for patients with chronic and complex conditions, especially older adults • Strong clinical judgment and ability to synthesize incomplete information into clear plans • Comfortable using technology and excited about leveraging AI to improve care delivery • Passionate about mental health and holistic care
