• Deliver comprehensive remote assessment, monitoring and care coordination for consumers with chronic and complex health conditions. • Identify and respond to early signs of clinical deterioration, escalating care and coordinating timely interventions when required. • Collaborate with consumers, carers and multidisciplinary healthcare providers to deliver high-quality, person-centred care. • Contribute to quality improvement initiatives, service outcomes and continuous professional development while participating in a 7-day roster.
• Current AHPRA registration as a Registered Nurse with a Bachelor of Nursing qualification. • Demonstrated experience in chronic disease management, comprehensive clinical assessment and person-centred care planning. • Strong communication, organisational and clinical decision-making skills, with the ability to work effectively within a multidisciplinary team. • Experience working in community or home-based healthcare settings, with sound computer skills and a current driver's licence.
