• Manage a caseload of community-based older Australians receiving government-funded aged care services in their homes • Assess clients’ needs and develop personalised care plans • Manage clients’ funding budgets according to government program guidelines • Onboard new clients and proactively manage existing clients • Use a compassionate, person-centred approach • Collaborate with clinical, scheduling and service delivery teams • Visit aged care clients in their homes • Communicate regularly with internal teams to ensure a smooth and comprehensive care experience
• Experience in community-based case management is ideal • Experience managing government-funded in-home aged care services and budgets is an advantage • Current driver’s licence • Reliable vehicle with minimum Third-Party insurance • Strong verbal and written communication skills • Demonstrated empathy and compassion • Excellent problem-solving and critical-thinking skills • Good organisational and time-management skills • Cultural competence and ability to work with diverse populations • Advocacy skills to ensure clients’ needs are met • Collaboration and teamwork skills for interdisciplinary teams • Diploma or Cert IV in Case Management or equivalent (desirable) • Successful candidates must complete a National Police check • Successful candidates must complete a Work Rights check • Successful candidates must confirm COVID-19 vaccination status
