📋 Description

• Provide one-to-one, person-centered support • Assist with daily living tasks including personal care, meals, transportation, housekeeping, and laundry • Maintain a safe, comfortable home environment • Teach and build skills in community participation, health and wellness, safety, communication, social interaction, and household management • Promote problem-solving and self-advocacy through education • Administer and document medications and provide health-related supports • Encourage positive behavior and follow individualized behavior support plans • Respond to crises using prevention, intervention, and safety techniques • Document services and progress accurately • Manage a caseload of 20+ individuals

🎯 Requirements

• Proficient with Microsoft Office software (Outlook, Word, etc.) • Ability to learn case management software • Current, valid MN driver’s license • Safe, legal driving habits • Compliance with Rule 11 and MVR background check required
