📋 Description

• Conduct virtual wellness visits directly to patients in the comfort of their homes. • Leverage medical expertise to offer a wide range of healthcare services. • Make warm and welcoming connections with a diverse range of patients. • Engage in proactive care by conducting health risk assessments and educational informational sessions. • Provide comprehensive care coordination and management, including preventive care interventions and medication management.

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in any of the following states: KY, MI, OH, IL • Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services • Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking • Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search
