📋 Description
• Conduct virtual wellness visits directly to patients • Review case load and prepare technology • Navigate through schedule and partner with in-home team • Make meaningful connections with patients • Engage in health risk assessments and medication reviews • Provide comprehensive care coordination and management
🎯 Requirements
• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Consistently exhibits highest levels of professionalism • Willingness to work in revolutionary environment • Technologically savvy • Strong written and verbal communication skills • Ability to work independently or in a team • Pass national background check and valid clinical license search
