đ Description
⢠Complete remote order entry, order verification, and drug verification. ⢠Transitions of care medication reconciliation. ⢠Navigate multiple pharmacy information systems (PIS). ⢠Provide drug information and therapeutic guidance to providers. ⢠Assess medication appropriateness, dosing (including renal/hepatic adjustments), interactions, and compatibilities. ⢠Support specialized populations such as pediatrics, geriatrics, pregnant/nursing patients, HIV, and those with renal/hepatic impairment. ⢠Maintain knowledge of IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, chemotherapy, and more. ⢠Document interventions and clinical actions accurately and promptly. ⢠Communicate effectively with physicians and nurses. ⢠Ensure compliance with HIPAA, pharmacy laws, and client policies.
đŻ Requirements
⢠Bachelors in Pharmacy required; PharmD preferred ⢠At least 1 year of hospital pharmacy experience required ⢠Active and in good standing IL state pharmacist license required ⢠Willingness to obtain additional state licenses (IA, NE, HI) ⢠Strong clinical knowledge and comfort working across multiple systems ⢠Ability to work hours that may include evenings, overnights, weekends, and holidays
